Patient Description

A 48-year-old male has been experiencing recurrent irritation, swelling, moisture, discoloration, and extreme itchiness on the penis and scrotum for approximately 4 months. These symptoms occur about 3 times a week, lasting for a day each time. The episodes start suddenly and are followed by the skin cracking and bleeding as the swelling subsides and the moisture dries up. The patient finds hydrocortisone somewhat helpful. Desitin was also tried with some success, but triple antibiotic ointment led to extreme itchiness and further inflammation. The patient reports that applying lotion of any kind triggers a flare-up. There is no deep tissue pain, only skin discomfort due to cracking and peeling. The affected area is moist with clear fluid, red, and warm but there is no unusual discharge.
