[40F] With severe chest pain, shortness of breath, and radiating pain

Patient Description

A 40-year-old female has been experiencing severe, sharp, and throbbing chest pain since last Saturday. The pain has gradually worsened and now extends to her neck, left arm, and back left shoulder blade, and has also started moving to the right side of her chest. The chest pain makes it very difficult for her to breathe or talk, and it is so severe that it takes her breath away. A 10 mg pain pill provided by a coworker slightly eased the pain, but it returned to severe levels within two hours. She has also been experiencing shortness of breath for the past three days, which she rated as 7 out of 10 in severity. Additionally, she has felt nausea, light-headedness, and alternating between feeling freezing and sweating. She has not had any fever, but has noticed swelling in her ankles. She has cervical cancer and is very anemic, but the medications for anemia make her very sick, so it hasn't been treated recently. She smokes cigarettes but does not use alcohol or recreational drugs. Her family history includes congestive heart failure, diabetes, and high blood pressure, although she has not been diagnosed with these conditions herself. She has had a tubal ligation, several ovarian cysts removed, and her gall bladder and appendix removed. She is allergic to Finergan but is not currently taking any medications.

What might be happening

Based on the symptoms you've described, there are several serious conditions that could be causing your chest pain and other symptoms:

  1. Heart Attack (Myocardial Infarction): The severe chest pain radiating to your neck, left arm, and back, along with shortness of breath, nausea, and sweating, are classic signs of a heart attack. This is a medical emergency that requires immediate attention. Heart attacks occur when the blood flow to a part of the heart is blocked, often by a blood clot.

  2. Pulmonary Embolism: This condition occurs when a blood clot travels to the lungs, causing chest pain that worsens with breathing, shortness of breath, and sometimes swelling in the legs. Given your symptoms and the severity, this is another possibility that needs urgent evaluation.

  3. Aortic Dissection: This is a serious condition where there is a tear in the wall of the major artery carrying blood out of the heart (aorta). It can cause severe chest pain that radiates to the back and other areas.

  4. Pericarditis: Inflammation of the lining around the heart can cause sharp chest pain that worsens with breathing. This condition can also be serious and requires medical evaluation.

What you can do

Given the severity and nature of your symptoms, it is crucial that you seek immediate medical attention. Here are the steps you should take:

  1. Call 911: Given the potential for a heart attack or other serious conditions, you should call 911 immediately. Do not attempt to drive yourself to the hospital.

  2. Emergency Room: If for any reason you cannot call 911, have someone else drive you to the nearest emergency room as quickly as possible.

  3. Stay Calm: While waiting for emergency services, try to stay as calm as possible. Sit down and rest to avoid putting additional strain on your heart.

  4. Avoid Eating or Drinking: Until you are evaluated by a medical professional, avoid eating or drinking anything, as this could complicate potential treatments.

Remember, I am not a doctor, and this advice is based on the symptoms you've described. It is essential to get evaluated by a healthcare professional immediately to receive a formal diagnosis and appropriate treatment.

People Also Ask

What are the common symptoms of a heart attack in women?

Common symptoms of a heart attack in women include chest pain or discomfort, pain in the neck, jaw, shoulder, upper back, or abdomen, shortness of breath, nausea or vomiting, sweating, lightheadedness, and unusual fatigue. These symptoms can be more subtle in women compared to men.

How can smoking affect heart health and increase the risk of heart-related conditions?

Smoking damages the lining of the arteries, leading to a buildup of fatty material (atherosclerosis) which can cause heart disease. It also increases blood pressure and heart rate, reduces oxygen to the heart, and increases the risk of blood clots, all of which can lead to heart attacks and other cardiovascular conditions.

What is the relationship between anemia and shortness of breath?

Anemia can lead to shortness of breath because it results in a reduced number of red blood cells or hemoglobin, which are responsible for carrying oxygen throughout the body. When the body doesn't get enough oxygen, it can cause symptoms like fatigue and shortness of breath.

Why is it important to seek immediate medical attention for severe chest pain?

Severe chest pain can be a sign of life-threatening conditions such as a heart attack, pulmonary embolism, or aortic dissection. Immediate medical attention is crucial to diagnose the cause and begin appropriate treatment to prevent serious complications or death.
