Patient Description

A 28-year-old female reports experiencing extreme pain in the stomach, primarily localized below the belly button, sometimes shifting to the left side and rarely to the right. The pain is described as sharp and stabbing, sometimes feeling like a muscle spasm, and is accompanied by a dull ache. The pain can be so intense that it wakes her up at night and makes it difficult to move or walk. The patient has been experiencing irregular vaginal bleeding and spotting for the past two months, which has worsened recently. The discharge is often blood-like, ranging from pink to bright red, and is almost constant. This irregular bleeding started before a recent assault. Additionally, the patient has been struggling with constipation, alternating with very loose stools, and experiences pain during bowel movements. The stool is often dark. She has had some pain during urination, but no fever, unusual vaginal discharge, or pain during intercourse. She has experienced some nausea but no vomiting or changes in appetite. The patient has a history of Graves' disease, diagnosed in 2016, and is allergic to penicillin, amoxicillin, azithromycin, and ciprofloxacin. She is not currently taking any medications. She also mentioned using methamphetamine daily since February.
