[22M] Experiencing urinary retention and frequency, cold sweaty feet, and eye discomfort

Patient Description

A 22-year-old male has been experiencing urinary urgency and retention, with a need to urinate every 20 minutes or less, resulting in only small amounts of urine and a sensation of incomplete emptying. These symptoms began suddenly 3-4 days ago, with a day of relief in between. The patient's urine is dark yellow, but no change in odor has been noted. Additionally, the patient reports cold, sweaty feet and a weird sensation in the feet and genital area, as well as mild burning discomfort in the eyes for the past 2 days. The patient has used cocaine recently. There is no history of trauma, allergies, or pre-existing conditions, and the patient is not currently on any medications. No family history of similar symptoms is reported.

What might be happening

Based on the information you provided, possible causes of your symptoms include:

  • Urinary Tract Infection (UTI): This is an infection in any part of your urinary system, which includes your kidneys, bladder, ureters, and urethra. UTIs are more common in women, but men can get them too. Symptoms can include a strong, persistent urge to urinate, a burning sensation when urinating, passing frequent, small amounts of urine, and urine that appears cloudy or strong-smelling.

    • Treatment usually involves antibiotics to kill the bacteria causing the infection.
  • Prostatitis: This is swelling and inflammation of the prostate gland, a walnut-sized gland situated directly below the bladder in men. Acute bacterial prostatitis is often caused by common strains of bacteria. Symptoms can include difficulty urinating, painful urination, and flu-like symptoms.

    • Treatment usually involves antibiotics to kill the bacteria causing the infection.
  • Drug use: Cocaine use can cause a variety of urinary symptoms. Cocaine can cause significant damage to the urinary tract and kidneys, leading to symptoms such as urinary urgency, frequency, and retention.

    • Treatment involves discontinuing the use of cocaine and managing symptoms.

What you can do

  1. Drink plenty of water: This can help dilute your urine and ensure that you'll urinate more frequently — allowing bacteria to be flushed from your urinary tract before an infection can begin.
  2. Avoid drinks that may irritate your bladder: Avoid coffee, alcohol, and soft drinks containing citrus juices and caffeine until your infection has cleared. They can irritate your bladder and tend to aggravate your frequent or urgent need to urinate.
  3. Use a heating pad: Apply a warm, but not hot, heating pad to your abdomen to minimize bladder pressure or discomfort.

What you should look out for

You should seek immediate medical attention if:

  • Your symptoms persist or worsen
  • You develop a fever
  • You notice blood in your urine
  • You experience severe pain in your lower abdomen or back

Please remember that this advice is intended to be general in nature, and specific causes may not apply to your situation. Always consult with a healthcare provider for appropriate examination, testing, and care recommendations.
