[21F] Experiencing persistent breast tenderness and lump with systemic symptoms

Patient Description

A 21-year-old female has been experiencing persistent tenderness in her breasts, particularly pronounced in the right breast where a lump approximately the size of a clementine is present. This lump, located near the center cleavage area, has been associated with a heavy pulling sensation, constant pain, and occasional excruciating burning pain extending to the nipple and armpit. The symptoms have been worsening over the past 2 years, with recent months seeing an increase in severity. Skin over the lump often becomes itchy, red, and feels numb yet painful. Additional symptoms include frequent headaches or migraines, night sweats resembling menopausal symptoms, difficulty breathing, and low-grade fevers ranging from 99.5 to 100°F. The patient has also experienced a significant unexplained weight gain of 50lbs over the past year. Past medical history includes surgery a year ago for the removal of benign ovarian cysts, and a recent overdose attempt 2 months ago with lamotrigine. The patient has allergies to Seroquel and sulfa drugs and has a family history of ovarian, uterine, and bladder cancer but no known breast cancer history. No current medications are being taken.

What might be happening

Based on the information you've provided, here are some possibilities:

  • Fibrocystic Breast Changes: This condition is characterized by breast pain, cysts, and noncancerous breast lumps. It's most common in women of childbearing age, but can affect women of any age. The condition often becomes less problematic after menopause.

    • Treatment typically involves managing symptoms. Over-the-counter pain relievers and supportive bras can help. In severe cases, your doctor may suggest surgery to remove a particularly painful or large cyst.
  • Breast Cancer: Given the size, location, and your description of the lump, as well as your other symptoms (night sweats, weight gain, fevers), this is a possibility that must be considered. Breast cancer can also cause skin changes and discomfort in the armpit area.

    • Treatment options for breast cancer include surgery, radiation therapy, chemotherapy, hormone therapy, and targeted therapy.
  • Mastitis: This is an infection of the breast tissue that results in breast pain, swelling, warmth and redness. It can also cause flu-like symptoms such as fever and chills.

    • Treatment typically involves antibiotics. In some cases, an abscess might need to be drained.

Please note that these are possibilities based on the symptoms you've shared and are not a definitive diagnosis. It's important to consult with a healthcare provider for a proper evaluation.

What you can do

  1. Schedule an appointment with a healthcare provider as soon as possible. They can perform a physical examination and possibly order imaging tests like a mammogram or ultrasound to better understand what's causing your symptoms.
  2. In the meantime, over-the-counter pain relievers can help manage your pain. Wearing a supportive bra might also provide some relief.
  3. Keep track of your symptoms. Note any changes in the size or feel of the lump, as well as any new symptoms.

What you should look out for

Seek immediate medical attention if:

  • You notice a sudden increase in the size of the lump
  • Your symptoms significantly worsen
  • You develop new symptoms such as nipple discharge, severe pain, or a high fever.

Because you're so young, your doctors might just tell you not to worry but if you haven't already gotten an ultrasound or diagnostic mammogram, advocate for yourself to get one so that you can figure out what this lump is. If it is a cyst, it can be drained. If it is some sort of mass (most likely benign) you can get it surgically removed if its bothersome. I work in a major surgery department and this week I was part of an operation on a young woman in her 20s with a benign breast mass.

No, typically cysts and simple fibrous masses shouldn't cause you to feel sick with such bothersome symptoms. The more worrisome causes at this point are an abscess or mastitis (infection). Obviously you would also want to rule out breast cancer, but that would be exceptionally rare at this age. Either way, you do want to get checked out ASAP, sorry if I wasn't clear in my previous comment. Have you seen any doctors since the symptoms worsened?

every time i mention my worries, they’re looked past. my doctor is a women so i thought she might understand, but she said im too young for insurance to cover a mammogram. i have had a lot of problems with my hormones from a young age. my cup size grew rapidly and i was in a dd before the 8th grade, now within just a year, ive gone up to a ddd/f. before my od attempt a few months ago, i noticed things were getting bigger and hurting more. headaches increased, my weight jumped, i get dizzy and out of breathe, and more recently a really bad chest pain in the middle of my chest and i’ve never had that before. the night sweats are what bothers me the most because i never used to sweat. i live in a small town though and the surrounding towns around me are small compared to most big cities. the closest, in network doctor, would be almost 3 hours away so it’s hard to find anyone to look at it for me.

I see, I'm sorry to hear about how inconvenient getting care is for you. Some of what you mentioned could certainly be hormonal, but I really think these recent worsening symptoms are worrisome enough that the 3 hour trip will be worth if you can find transportation. If the lump has become infected you can be treated by antibiotics. If it is non-infectious but adequately bothersome you can have it surgically removed. There are treatments but if you wait too long it could be dangerous for your health
