[49F] Evaluation of missed periods, PCOS, leukocytosis, and weight gain

Patient Description

A 49-year-old female presents with concerns about missed periods, diagnosed PCOS, persistent leukocytosis, and recent weight gain. Over the last 8 months, she has gained 22 pounds, which she attributes to increased consumption of regular soda, from 2 to 4 cans daily. Her periods stopped 3 months ago, and she has a long-standing history of irregular periods due to PCOS. Additionally, she reports always having slightly elevated white blood cell counts, around 13,000, for the past 10 years. She is currently managing degenerative disc disease with a history of a C5-C7 plate and laminectomy. Her current medications include Venlafaxine, Topiramate, Adderall, Oxycodone, Vitamin D, and Ibuprofen.

What might be happening

Based on the information you provided, the following conditions could be contributing to your symptoms:

  • Polycystic Ovary Syndrome (PCOS): This is a hormonal disorder common among women of reproductive age. Women with PCOS may have infrequent or prolonged menstrual periods or excess male hormone (androgen) levels. This can result in weight gain, acne, and excessive hair growth (hirsutism), all of which you have reported.

    • Treatment for PCOS often involves lifestyle changes like a healthy diet and regular exercise. Medications may also be prescribed to manage individual symptoms like menstrual irregularities and hirsutism.
  • Menopause: You mentioned that you are 49 years old and your periods have stopped. This could be a sign of menopause, which typically occurs in women between 45 and 55 years of age. Symptoms can include hot flashes, night sweats, mood changes, and weight gain.

    • Treatment for menopause symptoms can include hormone replacement therapy (HRT), as well as lifestyle changes and over-the-counter remedies.
  • Leukocytosis: This is a condition characterized by an increased number of white blood cells. It is often a response to an infection or inflammation, but can also be caused by certain medications, stress, or underlying conditions like leukemia.

    • Treatment for leukocytosis depends on the underlying cause. It's important to follow up with your healthcare provider to monitor this condition.

Please note that this is not a definitive diagnosis, and it's important to consult with a healthcare provider for a formal assessment.

What you can do

  1. Consider reducing your soda intake: Regular soda is high in sugar and calories, which can contribute to weight gain. Try replacing soda with healthier alternatives like water, unsweetened tea, or flavored sparkling water.

  2. Maintain a healthy diet and regular exercise: This can help manage weight gain and improve overall health. Aim for a balanced diet rich in fruits, vegetables, lean proteins, and whole grains. Regular physical activity can also help manage weight and improve mood.

  3. Consult with a healthcare provider: It's important to discuss your symptoms with a healthcare provider who can provide a formal assessment and recommend appropriate treatment options.

What you should look out for

  • If your symptoms worsen or if you develop new symptoms, it's important to seek medical attention.
  • If you experience severe abdominal pain, heavy vaginal bleeding, or severe mood changes, seek immediate medical attention.
  • If your leukocytosis worsens or if you develop symptoms like fever, chills, or unexplained weight loss, consult with a healthcare provider as soon as possible.

Remember, it's important to consult with a healthcare provider for a formal diagnosis and treatment. This information is intended to be educational and should not be used as a substitute for professional medical advice.

Lat year at age of 29, I received a diagnosis of PCOS Currently, I weigh around 207 lbs. Over the past 8 weeks, I have made significant changes to my diet, but I have barely lost any weight.
