Patient Description

A 45-year-old female presents with dry, swollen ankles causing pain for the past 4-5 months. The swelling worsens by the end of the day, and the skin around the ankles has become darker and thicker. The patient also reports multiple joint pain, including sharp pain in the right knee when bending or exercising, and continuous throbbing pain in the right elbow that worsens with lifting items or exercising. Both pains are rated 7-8 out of 10 on most days. Additionally, the patient has extremely dry hair, particularly on the sides, which breaks off when combed, and extremely dry skin that cracks in cold weather. She experiences constant constipation with large bowel movements twice a week for over a year. The patient feels unusually tired and has noticed weight gain that is difficult to lose and maintain.
