[42M] With foot and ankle pain post-Covid-19

Patient Description

A 42-year-old male reports experiencing throbbing pain in the arch of both feet and in the left ankle. The pain in the feet started today and occurs intermittently, while the pain in the left ankle began 5 days ago. The ankle was swollen on the first day, but the swelling subsided after that, although the pain persists. The pain worsens with standing and improves with sitting or lying down. Over-the-counter pain relief has been ineffective, but lidocaine patches on the ankle provide some relief. The patient tested positive for Covid-19 about 7 days ago and experienced fever and fatigue on the first day, which have since subsided. There have been no injuries that could have caused the pain. The patient denies any other symptoms, joint pain, muscle pain, numbness, or tingling. No history of autoimmune diseases or chronic illnesses. The patient takes daily multivitamins and occasionally uses ibuprofen and Mylanta for heartburn. No known drug allergies and no use of topical medications.

What might be happening

Based on the information you have provided, here are some possible explanations for your symptoms:

  • Post-Viral Arthralgia: After a viral infection like Covid-19, it's not uncommon to experience joint and muscle pain. This condition is known as post-viral arthralgia. The pain can be throbbing and may worsen with activity.

    • Treatment: Typically, this condition is managed with rest, over-the-counter pain relievers, and sometimes physical therapy. The pain usually resolves on its own over time.
  • Plantar Fasciitis: This is a common cause of pain in the arch of the foot. It occurs when the plantar fascia, a thick band of tissue that runs across the bottom of your foot, becomes inflamed.

    • Treatment: Rest, ice, stretching exercises, and over-the-counter pain relievers can help. Wearing supportive shoes and using orthotic inserts may also provide relief.
  • Tendinitis: The pain in your left ankle could be due to tendinitis, which is inflammation of the tendons. This can happen after an injury or overuse.

    • Treatment: Rest, ice, compression, and elevation (RICE) are commonly recommended. Over-the-counter pain relievers and physical therapy can also be helpful.

What you can do

While it's important to consult a healthcare professional for a formal diagnosis and treatment plan, here are some steps you can take to manage your symptoms:

  1. Rest and Elevate: Try to rest your feet and ankle as much as possible. Elevating your feet can help reduce pain and swelling.
  2. Ice Therapy: Apply ice packs to the painful areas for 15-20 minutes several times a day to reduce inflammation.
  3. Supportive Footwear: Wear shoes with good arch support and cushioning to alleviate pain in the arches of your feet.
  4. Stretching Exercises: Gentle stretching exercises for your feet and ankles can help relieve pain and improve flexibility.
  5. Over-the-Counter Pain Relievers: Continue using over-the-counter pain relievers like ibuprofen, but follow the recommended dosage.
  6. Use MDandMe: You can use MDandMe to journal your symptoms, track your progress, get insights, and share your medical notes with your healthcare provider.

What you should look out for

Seek medical attention if you experience any of the following:

  • Increased Pain or Swelling: If the pain or swelling in your feet or ankle worsens or does not improve with rest and home care.
  • Fever or Chills: If you develop a fever or chills, as this could indicate an infection.
  • Difficulty Walking: If the pain becomes so severe that you have difficulty walking or bearing weight on your feet or ankle.
  • New Symptoms: If you develop any new symptoms, such as numbness, tingling, or changes in skin color.

Remember, while these suggestions can help manage your symptoms, it's important to consult a healthcare professional for a formal diagnosis and appropriate treatment.
