[38F] Experiencing severe abdominal pain with fever and vomiting

Patient Description

A 38-year-old female reports severe abdominal pain in the lower right quadrant, which began around the belly button and shifted to the lower right area, near the appendix. The pain, described as sharp and constant, has persisted for two days and is exacerbated by touch. Accompanying symptoms include fever, vomiting, and rebound tenderness in the affected area, but no radiation of pain, changes in urination, or abnormal bowel movements were reported. The patient's last bowel movement was described as normal. The patient has a history of high blood pressure diagnosed in 2006 and is currently on daily doses of Buspar (10mg), Wellbutrin (300mg), and ibuprofen (500mg). Notably, the patient has allergies to Tegretol and Trileptal. No family history of gastrointestinal diseases is reported, and no other symptoms were mentioned.

What might be happening

Based on the information you've provided, the most likely diagnosis is:

  • Appendicitis: This is an inflammation of the appendix, a small pouch attached to the large intestine. It typically starts with pain near the belly button that moves to the lower right side of the abdomen, which matches your symptoms. It's often accompanied by fever, nausea, and vomiting. The pain can become severe and is usually made worse by moving, taking deep breaths, coughing, or sneezing. The fact that you have rebound tenderness (pain when pressure is released) is also a classic sign of appendicitis.
    • Treatment for appendicitis usually involves surgery to remove the inflamed appendix. Before surgery, you may receive a dose of antibiotics to treat infection.

What you can do

Given the severity of your symptoms and the potential for complications if appendicitis is left untreated, it's important that you seek immediate medical attention. Please go to the emergency department right away.

What you should look out for

If you experience any of the following symptoms, they could indicate a serious complication related to appendicitis and you should seek immediate medical attention:

  • Increased intensity of abdominal pain
  • Abdominal swelling
  • High fever
  • Inability to pass gas
  • Persistent vomiting

People Also Ask

What are the common symptoms of appendicitis?

Common symptoms of appendicitis include abdominal pain that starts near the belly button and moves to the lower right side, fever, nausea, vomiting, and rebound tenderness. The pain often worsens with movement, deep breaths, or coughing.

How is appendicitis typically treated?

Appendicitis is typically treated with surgery to remove the inflamed appendix, known as an appendectomy. Before surgery, antibiotics may be administered to treat any infection.

What are the risks of delaying treatment for appendicitis?

Delaying treatment for appendicitis can lead to serious complications, such as a ruptured appendix, which can cause peritonitis (infection of the abdominal lining) or an abscess. These conditions can be life-threatening and require immediate medical attention.

Can medications like ibuprofen affect appendicitis symptoms?

Medications like ibuprofen can mask pain and inflammation, potentially delaying diagnosis and treatment of appendicitis. It's important to seek medical advice if appendicitis is suspected, rather than relying on pain relief alone.

Have you previously had your appendix removed? Or any imaging during this recent episode? Otherwise definitely go to the emergency room and get treatment as appendicitis if left untreated can lead to life-threatening complications.
