Patient Description
A 22-year-old female presents with a constellation of symptoms including neck stiffness and pain, missed menstrual period for 1.5 months, severe constipation followed by watery diarrhea with a strong, fishy smell, and vaginal bleeding with dark red spots. The patient reports an untreated urinary tract infection with cloudy, foul-smelling urine and mild pain during urination. She also experiences cold feet with purple spots on the skin, loss of appetite, weight gain despite trying to eat, and insomnia affecting her daily life and mood. Painful intercourse and a sensation of a ball inside her that may have ruptured, leading to bleeding, were also noted. Additionally, she has an infected belly piercing with a yellow scab, a low to medium-grade fever for several days, a sore throat that feels like it's closing, and nasal congestion in one nostril for about a week and a half. Other symptoms include soreness under the arms, a rash on both sides of the jawline near the ears, ear drainage on the left side, numbness and tingling in the left leg after standing up, visual disturbances (seeing things that aren't there, black spots, purple spots, and sometimes yellow spots), dizziness, confusion attributed to memory loss, shortness of breath almost daily, and chest pain that comes and goes, usually feeling tight. The patient reports daily methamphetamine use and has experienced heart palpitations or irregular heartbeats. She has a family history of congestive heart failure and has had abnormal EKG results but did not follow up with the referral. Chronic conditions include a urinary tract infection and a yeast infection. Allergies include adhesive tape and latex. Medications include Claritin for seasonal allergies, an inhaler for asthma attacks, and Flonase for allergies, although the patient does not use Flonase.
